Day 30

Hello All,

JaNell’s stomach is still very sore after the last surgery. It’s especially painful when they move her. Her breathing was somewhat labored this morning when the doctor checked in. She hasn’t had any coughing, but nausea is still a problem. Her face was a little flushed and she complained that her mouth was dry. The doctor commented that she liked to keep the room cold and pile on the blankets to keep warm.

The blood sample cultures have identified three bacteria, which are being treated with antibiotics. The infection is the result of the perforation in her intestine. The main treatment for the problem was accomplished by repairing the perforation in the intestine.

To prevent spreading bacteria to other patients in the hospital, the hospital staff “gown-up” (put on a disposable smock) when they enter JaNell’s room. Guests are not required to wear a gown. When Dr. Seville, an infectious disease specialist, who is a petite Asian lady, left the room she gave the gown a vigorous tug and ripped the gown off. I told her it reminded me of the way Hulk Hogan removed his tee shirt.

Her vital signs are good. The infection will by monitored by checking temperature and white blood cell count.

Day 31

Hello All,

JaNell had a good day today. Her vital signs were good. She got out of bed, walked around the room and sat in a chair. Robert and Kelly stayed with her today. I took the day off. Kelly is staying in the room with JaNell tonight. They plan to return to Las Cruces tomorrow.

Progress is slow, but there hasn’t been any major set backs, after the second surgery. We are praying she is on the right track to recovery now.

Robert and Kelly’s friend, Dewey Wilson, who works at Dallas Cowboy games as an equipment manager, got Jason Whitten to sign a game jersey for JaNell.

Day 32

Hello All,

JaNell got out of bed one time today. Daren, the physical therapist, got her out of and back into bed without her complaining about pain. Daren, who is from Minnesota, came in on Sunday at the special request of her doctors, you betcha. The distance she walked was limited. JaNell gets dizzy and short of breath when she is getting out of and into bed. Daren says, "You got a lotta stuff hangin'." (on your IV Pole)

The hole, which was repaired in her intestine, is still seeping, which is not unusual, they say. Checking the fluid, which is collected form the tube in her side, studying the white blood cell percent, is monitoring the situation. Infection is being treated using: Cubicin, Diflucan and Ampicillin, which are wide spectrum antibiotics.

Water retention has not only caused swelling of the hands, feet and ankles, but has now caused conjunctival chemosis of the eyes. Dr. Thelma Barnes came in this morning and gave JaNell an eye exam. She said there was no damage to her eyes and her retina was in good condition and had no macular regeneration.

During a period this morning, she clicked her pain button 12 times, but her prescription only allowed dispensing of pain medicine 10 times.

Partner in Crime: A vent tube on the GN tube makes a whistling sound, that drives JaNell crazy. Upon request, I supplied her with a piece of tape to bind up the tube. Later, I ratted JaNell out, and told the doctor about the fix. She approved, and left the tape on.

Late this afternoon she watched football, and said she felt good. Her vital signs are good and white blood cell count is lower today than yesterday.

Nurse Katy is back tonight, she is the lead nurse of the A team.

Are her White Blood Cells still the same or less?

It all depends on the MDs parameters for what he thinks is expected for the JP drain to be draining now. Some MDs use the standard of once there is 30-40cc or less in 24 hours then it can come out, but it is up to the MD.

You should now take state boards and get your RN or more than that your MD for you are really getting educated! (I ask a lot of questions - Told the nurse I'm just tryin' to impress my friends back home - didn't tell her I'm callin' in the Big Guns from Baylor.) terry

Day 33

Hello All,

JaNell had a busy day; she got out of bed on three different occasions. Occupational therapy consisted of walking to the sink where she brushed her teeth and washed her face. She did an exercise of walking twenty steps in place. JaNell did her own version of this exercise; put your little foot.
Daren, the physical therapist had her walk into the restroom, even though she has a catheter, just to get accustomed to life’s routine.

A friend of the nurse came in today and sang 10 spiritual songs to JaNell. She sang one verse from each song.

The incision of the second surgery was left open (secondary intention healing,) because of the internal bacterial infection. Not a pretty sight. Her doctor told her she would have a battle scar conversation piece.

Her GN tube was clamped today for four hours, to determine if her system could function without it. She is experiencing some nausea; so they will probable unclamp it.

She was feeling anxious this afternoon and was given some anxiety medication.
Today was a day of slow hard-fought progress, but no step backward.

I've seen some flakey stuff. One nurse, wondering why the bed wouldn't roll, got down on all fours, put her gloved hands flat on the floor, then got up and continued to put eye drops in JaNell's eyes. Another dropped JaNell's cannula on the floor, stepped on it, then put it in her nose. Two were standing over her, dressing her wound, with out wearing masks, talking and spraying saliva everywhere. Then she went over and touched the door handle, returned and continued packing JaNell's wound. I could go on and on...

Dr. Haawkinson came in this morning: JaNell had a rough night and couldn't / wouldn't talk to her. She ask me for my opinion. She was in a medicated fog all night with moaning and talking in her sleep. Dr is not in favor of anxiety drugs for people over 65. It was an ordeal to get a bath this morning. Hard to roll over on her side... Yelled, complained and begged... Found out the high blood pressure readings last night was caused by a faulty cuff. With a new cuff perfect there was perfect readings.

May do a CT scan today or tomorrow, "to see what is going on." Will talk to Dr. Wasif to see what he wants to do.

1) Didn't mention CT Scan.
2) Will give her some juice after the anxiety medication wears off.
3) Gave me an assignment: After drinking juice does it come up immediately (just gaging - not such a bad situation,) or does the juice stay in the stomach a while, and then come up (throwing up - not such a good situation.) Wants me to observe...
4) I went to the hall with him and brought up the subject of the affect of obesity. He said it has a great impact on the surgery and recovery, that along with the fact that she is 72. He is impressed that she is handling this as well as she dose. Needs to eat food to regain strength.
5) Needs to walk...
6) Anxiety med. not a good idea, will give her something, that has a short term affect, for sleep tonight.
** Clamping tube now and drinking cranberry juice @ 8:48.
** She looks better now. Bev. thinks so, too.

No, but had a bowl movement yesterday.

No food intake?

No Food - Just cranberry juice this morning, for the first time since surgery, to determine if GN Tube can be removed.
If she throws up after the juice has been in her stomach for a while - the tube stays.

What is a GN tube?

Tube through your nose that pumps the stomach. Tracy said she had a GN tube one time.

Darien Physical Therapy & Tina Occupational Therapy.

1) got out of bed - good
2) walked to bathroom with walker - good. No bowel movement.
3) Returning from bathroom could only walk three steps past door. Had to sit and rest. Short of breath.
4) Heart Rate 115
5) Blood pressure 134/67.
6) Her legs seemed to be strong, limiting factor was shortness of breath.
7) Oxygen level 96
8) Back in bed now, sleeping - feet propped way up - muttering occasionally.
9) Still keeping juice down.
